Before you come in
Once your appointment is booked, we email you a secure intake form to complete online. It takes just a few minutes and means we can make the most of your time in the chair. Here is what it covers.
Your details
Name, contact details and, for a child, their parent or guardian, so we know who we are seeing and how to reach you.
Medical and dental history
Any health conditions, medications and your dental background, so your care is safe and tailored to you.
Consent
Simple consent to examine and, later, to treat. Your information stays private and secure with us.
There is no live form to fill in here. The secure intake form is emailed to you once your appointment is booked, so it is linked to your visit and kept private.

How do I get the new patient forms?
Once your appointment is booked, we email you a secure intake form. You complete it online before you arrive, so there is nothing to print or post. If you would prefer to fill it in at the clinic, that is fine too, just arrive a little earlier.
Do I need a referral to book?
No. You can book your free Smile Assessment directly, no referral is needed. If your dentist has given you a referral letter you are welcome to bring it, but it is not required to see us or to start treatment.
Can I do the first appointment on Zoom?
Yes. Your free 20 minute Smile Assessment can be done in the clinic in Newmarket or over Zoom from anywhere in New Zealand. A video call suits people who live out of the area or have a busy schedule, and you can come in later for a full examination.
How long does the first visit take?
Your free Smile Assessment takes about 20 minutes, in the clinic or over Zoom. It is a relaxed chat about your smile and the options, with no obligation. A full diagnostic work-up with a written plan and exact quote is a separate, longer appointment.
Can children register?
Yes. For a child, a parent or guardian completes the intake form on their behalf and comes along to the first visit. It helps to bring any medical and dental history and details of any medications your child takes.
Is my information kept private?
Yes. Your intake form is sent securely and your details are kept private and confidential, in line with New Zealand privacy rules. We only use your information to give you safe, tailored orthodontic care.
